By submitting this form you :-

• confirm that the you are signing off on the final working document/artwork for this phase of the project
• understand that this document/artwork will be used by our team to start the next phase of the project
• understand that any changes to layout/function/design once the next phase has commenced may incur additional fees
• if signing off on visual design for print; you confirm that all text has been proofread and is correct, you have double checked telephone numbers, email addresses etc.
